On your first visit to our office, please bring:
- The completed forms that we mailed to you
- Your insurance cards
- Medications you are taking
- Your referral form (if required)
- Copies of all previous tests that have been performed. If you have had CT scans, MRIs or X-rays, bring the actual films along with the reports.
You can also download forms below and bring them with you to your appointment.
Patients under 18 must be accompanied by a parent or legal guardian.
If you must cancel your appointment, please call us at least 24 hours in advance. This allows us to give your time to another patient in need of our services.

If you have a medical emergency after office hours:
- Call 911 immediately for life-threatening emergencies.
- Please contact the Lakeshore ENT doctor on call as soon as possible - 586-779-7610.
- If an emergency room visit is required, when possible, use a hospital where Lakeshore ENT doctors are on staff.
Our physicians are on staff at St. John Hospital and Medical Center, Bon Secours Cottage Hospitals, and St. Joseph Mercy of Macomb Hospital.
If you require hospitalization, your attending physician will arrange your admission whenever possible. During your hospital stay, either your attending physician or one of our associate physicians will see you daily to ensure your comfort and recovery.
If you are hospitalized by another physician for other reasons, you can arrange to see your Lakeshore ENT physician by notifying your attending physician or calling our office.
