Your First Visit

On your first visit to our office, please bring:

  • The completed forms that we mailed to you
  • Your insurance cards
  • Medications you are taking
  • Your referral form (if required)
  • Copies of all previous tests that have been performed. If you have had CT scans, MRIs or X-rays, bring the actual films along with the reports.

You can also download forms below and bring them with you to your appointment.

Patients under 18 must be accompanied by a parent or legal guardian.


Cancellations

If you must cancel your appointment, please call us at least 24 hours in advance. This allows us to give your time to another patient in need of our services.

Emergencies

If you have a medical emergency after office hours:

  • Call 911 immediately for life-threatening emergencies.
  • Please contact the Lakeshore ENT doctor on call as soon as possible - 586-779-7610.
  • If an emergency room visit is required, when possible, use a hospital where Lakeshore ENT doctors are on staff.

If You Are Hospitalized

Our physicians are on staff at St. John Hospital and Medical Center, Bon Secours Cottage Hospitals, and St. Joseph Mercy of Macomb Hospital.

If you require hospitalization, your attending physician will arrange your admission whenever possible. During your hospital stay, either your attending physician or one of our associate physicians will see you daily to ensure your comfort and recovery.

If you are hospitalized by another physician for other reasons, you can arrange to see your Lakeshore ENT physician by notifying your attending physician or calling our office.

Office and Surgical Fees

Our office fees are based on the complexity of your medical problem.

We participate in most major insurance plans and you will be told if we participate in your particular health plan.

We also accept cash, personal checks, VISA, MasterCard and Discover.

Your insurance company will be billed for X-rays and other diagnostic tests performed on the day of your visit. You must pay any amount not covered by your insurance at the time of your visit.

If you require surgery and you have insurance coverage, we will bill your insurance company.

Each insurance plan has different reimbursement conditions (full payment, deductibles, co-pays). Ultimately, the full payment responsibility is yours.

If you require surgery but have no insurance coverage, please contact our surgical scheduling department at (586) 445-6213 to develop a payment plan.

If you have any questions regarding our fees, you may discuss them with our business office.

In order for our doctors to provide a complete and comprehensive medical opinion it is important that they review all your medical information. Please be prepared to provide this information to our office along with your current medical insurance information.

If you have any questions about these forms, our staff will assist you with your questions on the day of your visit.
